We focus on people! Our residents are family. Conveniently located just minutes from Exit 30 on the New York State Thruway, the Mohawk Homestead is an adult care residence for the elderly. We offer care and services for both respite (temporary) and long-term residents. Established in 1841, we are an adult care facility. Adult care facilities are for people needing assistance with Activities of Daily Living (ADLs) but wishing to live as independently as possible for as long as possible.
Adult care facilities exist to bridge the gap between independent living and nursing homes. Residents in adult care centers are not able to live by themselves but do not require constant care. Adult care facilities offer help with ADLs such as eating, bathing, dressing, laundry, housekeeping, and assistance with medications. Adult care is not an alternative to a nursing home, but an intermediate level of long-term care appropriate for many seniors.

Patrick J. Fraccola
My name is Patrick J. Fraccola, and I am the Administrator of the Mohawk Homestead. I was born and raised locally, in our neighboring town of Frankfort where I graduated from Frankfort-Schuyler Central School in 2012. I then pursued my degree in Nursing and am proud to say I am a graduate of St. Elizabeth’s College of Nursing, Class of 2015. I have recently completed my Bachelor’s Degree through Grand Canyon University, as well. My professional experience includes: Surgical Inpatient Staff Nurse at Bassett Hospital in Cooperstown, Special Care Staff Nurse at MVHS-St. Elizabeth’s Campus, and Unit Manager of the Sub-Acute Rehab of Lutheran Care in Clinton, New York.

Susan States "I am the Case Manager at The Mohawk Homestead, where I have worked for over 8 years. I am a graduate of Utica College of Syracuse University, with a Bachelor’s degree in Social Work. My professional experience includes: Jefferson County Mental Health Clinic, Jefferson, Lewis and Herkimer County HARC’s, Director of Social Services for many years at Valley Health Services in Herkimer and same position for several years at Alpine Rehabilitation Center in Little Falls. My proudest moments in life have been watching my 2 sons graduate with Master’s degrees in Engineering Management and Computer Sciences. I enjoy working with the frail elderly and their families as I feel that maybe I can make their lives a little happier and a little more comfortable".
Personal Care Supervisor
Laurie states "I am the personal care supervisor at The Mohawk Homestead. I came back to work here in February 2016. I have been an LPN for over 35 years and have worked in long term care for most of this time. I have always enjoyed working with the elderly. I have worked in a hospital setting, rehabs units, dementia units, home care, office and medical surgical units. I live in Little Falls with my husband Bill. I have three grandchildren which I enjoy on my time off from work. I love working at The Mohawk Homestead. One of the main reasons is of course the residents. I find that The Mohawk Homestead being a smaller facility provides that special interpersonal relationships with co-workers and residents. To provide the best care, it is a TEAM effort".
Director of Activities
Since December 2014, Elaine Mabry has been The Mohawk Homestead’s Director of Activities. She operates on the belief that all people are valuable and unique and strives to empower her residents through individualized, community based and meaningful group activities. She states she is blessed to serve seniors at The Mohawk Homestead in unique, fun, and creative ways. Elaine has brought valuable experience working in activities at a skilled facility and holds a B.S. in Health Services Management. Her and husband Dean reside in Ilion where they recently planted a church.
